Celiac Disease Center Online Survey
DO YOU HAVE CELIAC DISEASE OR GLUTEN SENSITIVITY?

The Celiac Disease Center at Columbia University is conducting a study on the health and nutrition practices of people with celiac disease and gluten sensitivity. To join the study, we ask that you take a few minutes to fill out a survey. Participation is voluntary and anonymous.

The study is for those 18 years or older only.

Please do not enter any information that may identify you (such as your name, date of birth, address, or social security number). There is no compensation for participating in this study. If you have any questions about the study, please contact Dr. Tennyson or Dr. Green at 212-305-5590. The Celiac Disease Center at Columbia University Medical Center was established within the Department of Medicine at Columbia University in 2001. The Center's mission is to redefine the future of celiac disease and treatment through continuing advances in patient care, research, education and patient advocacy.

The Celiac Disease Center at Columbia University Medical Center is one of a few centers in the United States that provides comprehensive medical care, including nutritional counseling, for adult and pediatric patients with celiac disease. All of the Center's research is directed toward celiac disease clinical, epidemiology, and mechanisms of pathogenesis of celiac disease and patient and physician education
